4.8 Provider Termination Policy

clinicians may choose to terminate their position as a hopscotch provider and remove themselves from the network at any time when deciding to leave the network, clinicians must provide hopscotch with a 30 day written notice and should make every effort to ensure that clients they are currently servicing have been appropriately transitioned to other services by communicating with hopscotch’s clinical operations and customer service teams hopscotch reserves the right to suspend or terminate a provider if there is any violation of the terms within hopscotch’s written policies and protocols, the independent contractor agreement, or for any other behavior deemed inappropriate hopscotch will have no liability to a provider after termination occurs, except as required by law (i e – hipaa)
