providers must have a completed and signed note for each session that occurs within the platform and is billed to the hopscotch provider network each note must be included in the session history for the specific client that it corresponds to within a provider's active caseload providers may include an addendum to their note if any summary information or diagnosis code is incorrect reasoning must be included within the note to identify why changes were made the addendum note must be signed and verified by the provider providers may not share notes with other medical or non medical parties within or external to the hopscotch provider network unless the client/caregiver provides a signed release of information specific to the clinician/non clinician with whom the information is being shared documentation of release will reside under the client profile and must be renewed and signed annually by the client and/or caregiver auditing hopscotch reserves the right to periodically audit clinical notes for our network providers on a recurring basis and for the purposes of insurance regulatory requirements hopscotch must adhere to the rules and regulations of our contracted insurance payors and will provide these organizational partners with clinician notes for audit upon individual organization request request for records by provider for any clients no longer active on the provider’s caseload, notes will be available to the provider upon request providers must send verbal or written communication to the clinical operations team to request archived client records requests will be fulfilled according to state guideline requirements or within 10 business days of written notification whichever is sooner by current client/caregiver or former client/caregiver a client/caregiver must send verbal or written communication to the clinical operations team to request archived client records requests will be fulfilled according to state guideline requirements or within 10 business days of written notification whichever is sooner by third party hopscotch manages third party record requests including but not limited to insurance audits, subpoena of records, etc hopscotch providers will notify the clinical operations team immediately upon receipt of a subpoena pertaining to a hopscotch client providers will immediately submit a copy of the subpoena to the clinical operations team upon receipt of such notification the clinical operations team alongside hopscotch’s legal counsel will review the subpoena and release the appropriate documentation/records being requested to the appropriate parties failure to notify hopscotch of legal requests for documentation will be considered grounds for immediate termination
