hopscotch providers must maintain an updated caqh credentialing profile through the course of their participation in the hopscotch provider network initial credentialing occurs for each provider upon hire and recredentialing will occur annually to maintain the most current up to date information including but not limited to licensure, liability insurance, speciality, board certification, practice location, etc providers are encouraged to re attest (ensure information remains accurate) every four months when notification is received from caqh providers must respond to all credentialing, recredentialing, and document verification requests from hopscotch’s clinical operations team within 2 business days to maintain timely communication and completion of requests failure to respond to requests for additional credentialing information may result in temporary suspension or removal from the hopscotch provider network
