1.5 Duty to Notify Hopscotch

providers have a duty to notify hopscotch in the event that provider’s license to practice has been suspended, surrendered, revoked, placed on probation, or otherwise made inactive a complaint or grievance has been filed in any capacity a summons or a subpoena pertaining to their professional practice has been received (refer to summons/subpoena policy guidelines) provider is charged with a crime (excluding minor traffic violations) a lapse in provider’s insurance coverage occurs for any reason an event occurs which is expected to expose hopscotch to a legal claim or negative publicity or interfere with provider’s inability to perform contracted services any other disciplinary or professional liability actions are initiated against provider if any of the aforementioned events were to occur, providers can notify hopscotch by contacting the clinical operations team at hopscotch reserves the right to temporarily suspend the provider’s public facing profile within the hopscotch directory, hold pending client consultation requests, and hold pending sessions until an internal review is complete and a decision has been rendered hopscotch reserves the right to immediately terminate a provider’s contract and remove them from the hopscotch provider network if notification of one or more of the aforementioned events is not received
